Athletics Forms

Students may not participate in any athletic events, including tryouts and practices, until all paperwork is on file with the Athletics Department.

• Sports physical: A sports physical must be completed and the form signed by a medical doctor. A stress test, eye exam, and urinalysis must be inlcuded in the physical.
• Parent/Legal Guardian Permission Form: This form must be filled out and signed by a parent if the student is under 18 years of age. *The student will only be able to participate in sports that are checked off on this form.
• Parent/Guardian/Student Information Form: MSSD has secondary insurance for athletic injuries incurred during sports practices and games. This form must be completed in order to process claims. A copy of the student's health insurance card must be included.
