PERSONNEL AND BUDGET MANAGEMENT
CHANGE OF ADDRESS OR CHANGE OF NAME
Change of address/change of name forms may be found and filled out at
Ms. Jacqueline Nolen's office at KDES 3202. These forms may also be found
on the Payroll Office's website. You may give your forms to Ms. Nolen
(KDES 3202) or Ms. Newman (KDES 3201).
- For name changes, attach a copy of your new social security card to
the form.
- If you move from one jurisdiction to another (from DC to MD, from
MD to VA, etc.), you will need to fill-out a new state tax form and
attach it to the form.
CURRENT LEAVE BALANCES
In addition to the bi-weekly leave report, which is sent to supervisors,
current sick leave balances for teachers and annual and sick leave balances
for staff are shown on employee paycheck check stubs.
PURCHASING
The procurement policy may be found on the Gallaudet website - Policy
# 2:06. Please check with your supervisor to discuss your planned purchase.
Please contact Ms. Jacqueline Nolen for assistance with Gallaudet University
forms and procedures required to request that a planned purchase be approved.

Rene' Newman for assistance with Gallaudet University forms and procedures

Employee time cards and Payroll Information Forms are submitted to Ms.
Nolen every two weeks, according to the due dates listed on the Payroll
Schedule. A current copy of the Payroll Schedule may be obtained from
Ms. Newman or Ms. Nolen.
Ms. Nolen always keeps an up-to-date selection of many forms used at
Gallaudet. Most payroll, personnel and accounting forms may be found on
departmental Gallaudet websites.
